As the parent or legal guardian of child named above, I hereby acknowledge and agree to the following terms and conditions in consideration for my child's participation in the Stallions Cricket Coaching Program.

Assumption of Risk: I understand and acknowledge that participation in cricket activities involves inherent risks, including but not limited to physical injury, illness, and property damage. I accept full responsibility for any such risks arising from my child's participation in the Stallions Cricket Centre (Academy).

Release of Liability: I hereby release, waive, discharge, and covenant not to sue the Stallions Cricket Centre (Academy), its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or property while participating in the Stallions Cricket Centre (Academy).

Medical Treatment Authorization: In the event of an emergency, I authorize the staff of the Stallions Cricket to obtain medical treatment for my child, including transportation to a medical facility, if deemed necessary. I understand that every effort will be made to contact me or another emergency contact listed below prior to any medical treatment.

Photo and Video Release: I grant permission for the Stallions Cricket to use photographs and/or videos of my child taken during academy activities for promotional and marketing purposes, without compensation to me or my child.

Parent/Guardian Agreement: I certify that I am the parent or legal guardian of the child named above and that I have read this waiver and fully understand its contents. I agree to be bound by its terms on behalf of myself and my child.
